Please bring your insurance card with you on your first visit. Failure to do so will result in you being fully responsible for all fees at the time of visit.

• Blue Cross Blue Shield
• Medicare

Please contact your insurance company to verify that we participate with your specific plan. Covered procedures will vary among insurance companies.

We are also happy to see patients from other insurance plans that have out-of-network benefits.

Patients are responsible for co-pays, deductibles and remaining balances.
